Pacifica MD (operated by 1382452 B.C. LTD) is a physician-led medical aesthetics and wellness clinic located in Qualicum Beach, British Columbia. This Privacy Policy explains how we collect, use, and protect personal information through our website at pacificamd.com, including any forms, booking links, and marketing communications connected to it.
Our privacy practices are governed by the Personal Information Protection Act (PIPA), British Columbia’s private sector privacy legislation. Dr. Elisabeth Crisci is also regulated by the College of Physicians and Surgeons of British Columbia (CPSBC), whose standards govern the handling of clinical patient information.
This policy covers information collected through our website only. Full clinical records — including your medical history, lab results, treatment notes, and prescriptions — are managed through Jane App, our secure practice management platform. Jane App maintains its own privacy and security certifications, including PIPEDA compliance. This policy does not govern information collected or stored within Jane App.
2. What We Collect on Our Website
Our website is primarily an information and inquiry platform. The personal information we collect through it is limited to:
Your name, email address, and phone number, submitted through our contact forms or waitlist sign-ups
Your email address, submitted when booking an appointment (which routes to Jane App)
Symptom and eligibility information submitted through our candidacy questionnaires for hormonal and metabolic care or women’s genitourinary health services
Referral information, where you choose to share Pacifica MD with a friend using our referral program
Technical and usage data collected automatically when you visit our website (see Section 5)
We do not collect payment information, clinical health records, prescriptions, lab results, or treatment documentation through our website.
3. Candidacy and Eligibility Forms
Our website includes candidacy questionnaires for our Hormonal and Metabolic Care program and our Women’s Genitourinary and Sexual Health service. These forms ask symptom-related questions to help determine whether our services may be appropriate for you.
We treat any health-related information submitted through these forms with a higher level of care, consistent with the sensitivity of that information. Submitting a candidacy form does not establish a physician-patient relationship and does not constitute medical advice or a clinical assessment.
These forms are hosted and processed by Monday.com, a US-based platform. Information submitted through these forms may be stored on servers outside of British Columbia or Canada, and would therefore be subject to the laws of those jurisdictions. For further information, see monday.com/privacy.
4. How We Use Your Information
We use personal information collected through our website to:
Respond to your inquiries and contact form submissions
Review your eligibility for our services and follow up by phone or email
Facilitate appointment bookings through Jane App
Send appointment reminders and non-marketing clinic communications
Send marketing emails, newsletters, and promotional offers where you have consented (see Section 6)

Our email marketing is managed through Constant Contact, a third-party email platform. By using Constant Contact, your email address is processed and stored on their platform, which operates under its own privacy policy.
Express Consent
We rely on express consent where you have actively opted in — for example, by checking a box on a form, subscribing to our newsletter, or completing our referral program and choosing to receive marketing emails.
Implied Consent
Where you have an existing business relationship with Pacifica MD, such as a recent booking or inquiry, we may rely on implied consent to send relevant communications for up to two years from your last interaction, in accordance with Canada’s Anti-Spam Legislation (CASL).
Unsubscribing
You can unsubscribe from marketing communications at any time by clicking the unsubscribe link in any marketing email, or by contacting us at privacy@pacificamd.com. Unsubscribing from marketing does not affect non-marketing communications related to your appointments, care, or account.
7. Referral Program
Pacifica MD offers a referral program that allows you to share a referral invitation with a friend. If you provide a friend’s email address through our referral form, you are confirming that you have their permission to do so.
Your friend will receive a one-time invitation email identifying you as the referrer. They will not be automatically subscribed to our marketing list unless they separately choose to do so. Referral credits are subject to the terms stated at the point of referral, and apply to aesthetic products and services only, not to medical programs.
8. VISIA Skin Analysis
Pacifica MD uses the VISIA Complexion Analysis System to capture high-resolution facial images and assess skin conditions as part of our aesthetic consultation process. VISIA images are clinical in nature and are used to inform your personalized treatment plan and to track changes over time.
VISIA images are stored within our clinical systems and managed in accordance with our records retention obligations. These images are not used for marketing purposes without your separate, explicit consent.
9. Booking, Third-Party Platforms, and Linked Websites
Our website links to several third-party platforms that operate independently of Pacifica MD:
Jane App (pacificamd.janeapp.com) — our appointment booking and practice management system. Information entered in Jane App is governed by Jane App’s own privacy policy.
Monday.com — used to host our contact forms and candidacy questionnaires. Information submitted through these forms is processed by Monday.com under their privacy policy.
Constant Contact — used to manage our email marketing list and send marketing communications.
AlumierMD (ca.alumiermd.com) — an external skincare product retailer. Purchases made through their site are governed by AlumierMD’s privacy policy.
12weekmetabolicreset.com — a separate website for Dr. Crisci’s 12 Week Metabolic Reset program, which operates independently of Pacifica MD. That program’s own privacy policy applies.
Pacifica MD is not responsible for the privacy practices of any third-party websites or platforms. We encourage you to review their privacy policies before submitting personal information.
10. Consent
By submitting information through our website forms, booking an appointment, subscribing to our newsletter, or participating in our referral program, you consent to the collection, use, and disclosure of your personal information as described in this Privacy Policy.
For candidacy forms involving health-related symptom information, your submission constitutes consent to use that information to contact you about our services and assess your eligibility.
You may withdraw your consent to marketing communications at any time (see Section 6). Withdrawing consent does not affect information already collected or your ability to receive non-marketing communications from us.
11. Retention of Information
We retain website-collected contact information (name, email, phone number) for as long as reasonably necessary to respond to your inquiry or maintain your marketing consent, or as required by law.
Clinical records maintained within Jane App are retained in accordance with the obligations applicable to physician practices in British Columbia. Under CPSBC standards, medical records are generally retained for a minimum of 16 years from the date of the last entry, or until the patient turns 19 years of age, whichever is later.
When personal information is no longer required, we take reasonable steps to securely delete or de-identify it.
12. Evolving Services and Website Scope
Pacifica MD is in the process of transitioning some services — including hormonal and metabolic care and women’s genitourinary health — to a separate website operated under Dr. Crisci’s name. During this transition, those services remain accessible through pacificamd.com and are covered by this Privacy Policy.
Patients who are transitioned to a new platform will be notified of any changes to applicable privacy practices at that time. Information collected while services were operated under Pacifica MD will continue to be governed by this policy unless you are specifically notified otherwise.
13. Security
Pacifica MD takes reasonable steps to protect personal information collected through our website against unauthorized access, disclosure, or loss. These measures include access controls, secure platforms, and staff confidentiality expectations.
No method of electronic transmission or storage is completely secure. While we take reasonable precautions, we cannot guarantee absolute security. In the event of a privacy breach that poses a real risk of significant harm, we will take appropriate steps to notify affected individuals and relevant authorities.
Please do not submit sensitive medical information, urgent clinical concerns, or emergency communications through our general website contact form, social media, or ordinary email. If you are experiencing a medical emergency, call 911.

Pacifica MD’s services are intended for adults aged 40 and older. We do not knowingly collect personal information from minors through our website.
